PLEASE NOTE THAT FOR 2020 LEADERCONNECT/AGM YOU WILL BE BOOKING
YOUR  
REGISTRATION FEE, MEALS & HOTEL ROOM
THROUGH OUR DISTRICT ONLINE REGISTRATION.


(The Best Western has given us an extra special discounted room rate if we book the accommodations directly through our District online registration process and submit a room list to them. Your hotel room booking also includes a full hot breakfast at the hotel. Remember the deadline to book a hotel room is January 20th. After that date you may still register online - registration fee & meals, but you will be responsible to book your own hotel accommodations, 
as we need to release our room block back to the hotel by the deadline date.)

Each congregation with lay delegates is required to complete this form and return to the District Office by FEBRUARY 14th, 2020. The purpose of this form is to certify the Lay Delegates as the selected and approved delegates by the Congregation for the purpose of representation at the Annual General Meeting for Canadian Church of God Ministries.
The form must be signed by the Church Chairperson.

CANCELLATION/REFUND POLICY
 
Refunds (less administrative fee of first nights fee for accommodation) will be granted to requests received by email at events@chog.ca prior to
January 20th, 2020. No refunds will be granted after January 20th, 2020 due to binding commitments between the Canadian Church of God Ministries and vendors. Substitutions permitted if unable to attend.

Exception:
- A medical illness precluding a participant from attending the event per a doctor’s recommendation (note required).
- A death of a family member or good friend resulting in the participant attending memorial/funeral activities.
- If Canadian Church of God Ministries is required to cancel an event, a refund will be issued; notwithstanding, any fees on deposits that it is unable to retrieve from third party vendors. Please be aware that we may need to cancel an event due to unforeseen circumstances. We will try our best to not cancel, but if it comes to that point we will transfer your registration fee to another date or refund where possible.

If Canadian Church of God Ministries invoices a registrant for unpaid fees following the event, a 5% administrative charge will be added.
